RODO information clause

We would like to inform you that:

(a) The administrator of your personal data is Hanna Bieńkowska, conducting business under the name HANNAH Hanna Bieńkowska, with its registered office in Piaseczno, 1 Kineskopowa St. building D, CELTIC PARK, 05-500 Piaseczno (hereinafter: "Hannah");

(b) Your personal data contained in this form will be processed for marketing purposes;

(c) Your personal data will not be used for purposes other than those for which it was originally collected;

(d) The transfer of your personal data is carried out on the basis of your voluntary consent, in particular it is not the fulfilment of a legal or contractual obligation;

(e) Your personal data will only be shared with Hannah's employees and associates;

(f) Your personal data will not be transferred to third countries outside the European Economic Area;

(g) Your personal data will be retained by Hannah for a period of 3 years after it has been provided to Hannah;

(h) Your personal data will not be subject to automated processing that could affect your legal situation or otherwise affect your rights and obligations.


You are entitled to:

(a) The right to request access to and rectification of your personal data - where the data is inaccurate or incomplete, pursuant to Article 13(2)(b) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation) (Official Journal of the EU. L No. 119) (hereinafter: "RODO");

(b) The right to request erasure of data - where the data are no longer necessary for the purposes for which they were collected; where consent to data processing has been withdrawn; where an objection to data processing has been raised; where data are processed unlawfully; where data must be erased in order to comply with a legal obligation; where data were collected in connection with the offering of information society services in relation to a child or where processing of personal data is carried out on the basis of consent or contract by automated means, pursuant to Article 13(2)(b) of the RODO;

(c) The right to object to the processing of personal data, pursuant to Article 13(2)(b) RODO;

(d) The right to data portability, pursuant to Article 13(2)(b) RODO;

(e) The right to request the restriction of processing - where the data subject disputes the accuracy of the data; where the data are processed unlawfully and the data subject objects to their erasure, requesting a restriction instead; where the controller no longer needs the data for its purposes, but the data subject needs them for the defence or assertion of a claim; where an objection has been lodged - until it is established whether the legitimate grounds on the part of the controller override the grounds for the objection, pursuant to Article 13(2)(b) RODO;

(f) The right to withdraw consent to the processing of data by ZTM at any time, in accordance with Article 13(2)(c) of the DPA, whereby the withdrawal of consent will not affect processing carried out on the basis of consent before its withdrawal, in accordance with Article 14(2)(d) of the DPA;

(g) The right to lodge a complaint with the supervisory authority competent for data protection matters, in accordance with Article 13(2)(d) RODO.
